Extended Interval Aminoglycoside Dosing in Cystic Fibrosis
Patients with cystic fibrosis may harbor relatively resistant gram-negative organisms (most commonly Pseudomonas aeruginosa). The following doing strategy is often used in adult cystic fibrosis patients admitted to NMH.
Aminoglycoside levels should be drawn 2 hours and 6 hours after the first dose.
Use these 2 levels to estimate the aminoglycoside half-life
The dosing interval should be adjusted based on half-life (dosing every 6 half-lives with a dosing interval no shorter than q 12 hrs as per example below:
Aminoglycoside 2 hour and 6 hour post dose levels should be measured at least weekly in CF patients on aminoglycoside therapy. Changes in clinical status may require more frequent aminoglycoside level measurements.
